A chronic condition, as defined by the CDC, is any condition that last 1 year or more and require ongoing medical attention, or limit activities of daily living or both. Call us to find out if your condition is qualified by Medicare.

Can I receive a diagnosis from home?

Yes, in many cases. During your consultation appointment, you’ll have the oppritunity to discuss your symptoms with an expert medical professional who may be able to diagnose your condition and suggest a treatment plan, without you ever having to step foot in a doctor’s office.

Can I get a prescription and refills from home?

Yes, our medical team can recommend medications, fill prescriptions, and offer refills sent straight to your home or to the nearest pharmacy for pickup.

Is remote patient monitoring hard to use?

How often do I need to speak with my doctor?

Our chronic condition care plans include monthly video calls with a doctor. However, any health irregularities that show up during remote monitoring may warrant more frequent calls.
